My name is Kate Rencher. I am a third-year OTD student at Touro University Nevada.

Exploring The Impact of Pelvic Floor Dysfunction: We’re inviting you to fill out a survey. This survey is completely voluntary. There are no negative consequences if you don’t want to participate. If you start the survey, you can always change your mind and stop at any time. The purpose of this study is to better understand how symptoms of pelvic floor dysfunction affect your day to day life. This survey will ask questions about the type and severity of symptoms and strategies used to manage symptoms No identifiable information will be collected. This survey is expected to take less than 10 minutes.

Agreement to Participate: Your participation is completely voluntary, and you can withdraw at any time. To take this survey, you must be: 18 years or older and have experienced symptoms any of the following symptoms of pelvic floor dysfunction such as urinary leakage, pelvic organ prolapse, pain with intercourse, or inability to hold urine or bowel. If you meet these criteria and would like to take the survey provide consent below to begin the survey.
